For your convenience, UK King’s Daughters Walk-In Care for Schools is offering flu vaccinations for students during school hours. If you would like your child to receive the flu vaccine at school, please complete the online Flu Shot Consent and Consent for Services forms at https://schoolhealthconsentweb.kdmc.net. Both forms must be submitted in order for your child to receive the vaccine.
Please ensure that both consents are completed by October 1st, 2025.
If you prefer paper copies of the consent forms, they are available upon request at your child’s school.
If you have any questions, please feel free to call (606) 474-1925.

Please join us for three community sessions on September 4th, September 25th, and November 6th.
